Requirements:
- Accomplished OV Application Form (please see poster)
- Original and 1 photocopy of the original Philippine passport
- If the Philippine passport is expired, the original and 1 photocopy of proof of PH citizenship, such as a valid PR card, valid visit/study/work permit, or dual citizenship certificate (Identification Certificate).
- Personal appearance for photo and biometric capturing.
