IMPORTANT REMINDERS

The Philippine Passport is at all times the property of the Republic of the Philippines. It must be surrendered upon demand by an authorized representative of the Philippine Government.

Alteration, addition, or destruction of contents is strictly prohibited. Any unauthorized change will render a passport invalid and will have accompanying penalties.

Using the passport as collateral is against the law and will automatically render the passport cancelled.

The information provided in the passport application form must be true and correct. The documents that will be submitted should be complete and authentic. Under the law, making false statements in the passport application form, furnishing falsified or forged documents in support thereof are punishable by law.


BEFORE APPLYING FOR A PHILIPPINE PASSPORT:
  • All applicants should secure an online appointment when applying for a passport (one appointment per applicant). Families should book an appointment for each family member including minors.
  • The public is advised to secure their passports prior to booking their flights. Flight bookings should ONLY be made if passports are still valid for at least six (6) months.
  • Confirmed appointment is required for all applicants
  • Applicants are reminded to be at the Consulate or Consular outreach venue 10 minutes before their appointment.
  • Personal appearance is required for all applicants.
  • Prepare all the originals and photocopies of the documentary requirements for passport application.
  • Please come in proper business attire (with collar and sleeves).
  • Applicants will be required to take off their eyeglasses, contact lenses and/or facial piercings (on the nose, eyebrows, lips) during photo, biometrics and data capture. Jewelry such as earrings/ear piercings and necklaces are also not allowed during photo capturing.
  • Facial expression should be neutral (no heavy make-up, eyeliner or false eyelashes) with mouth closed. Ears should be made visible and forehead must be cleared (bangs/hair fringes should not cover forehead or touch the eyebrows.)

AFTER APPLYING FOR A PHILIPPINE PASSPORT:
  • Approximate processing period is 8 to 12 weeks after application.
  • Check if all the data on the new passport are correct and affix your signature on page 3 of the passport before leaving the Consulate.
  • For minor applicants 13 years old and above, immediately affix the signature on the third page of the passport booklet upon receipt of the passport.
  • For minor applicants 12 years old and below, leave it blank.
  • Passports unclaimed after one hundred and eighty (180) days will be cancelled per Department Order No. 37-03.
  • Do not staple anything on the passport booklet especially the covers as it may damage the electronic chip and render the e-passport unreadable.

Passport Release

  • The new passport shall be available in approximately 8 to 10 weeks after the date of application.
  • A self-addressed prepaid return envelope (with tracking number) PER APPLICANT must be provided upon submission of passport application. The applicant may secure said prepaid envelope from Canada Post (Xpresspost envelope) or from a courier service of their choice.
  • The Consulate does not assume responsibility for passports/documents that may be lost or damaged in the mail.
  • If opted for pick up, the applicant will receive an email confirmation that their passport is available for pick up in approximately 8 to 10 weeks.

ADULT RENEWAL APPLICANTS (18 YEARS OLD AND ABOVE)
A. Core Requirements

1. Confirmed Online Appointment. Book an appointment in this link: www.passport.gov.ph

2. Personal appearance

3. Current ePassport with photocopy of date page

4. Original PSA-issued documents that will support the change of name.

  1. Marriage Contract;
  2. Annotated Birth Certificate;
  3. Annotated Marriage Contract to show annulment/divorce/court ordered instruction; and/or
  4. Certificate of Death of Spouse

5. Original and photocopy of any of the following proof of current immigration status in Canada :

  • valid Canadian Permanent Residency card
  • valid visitor visa
  • valid work permit

6. Any valid government issued ID as secondary identification (i.e. PR card, driver’s license, Alberta ID, etc.)

7. Processing fee of CAD 93.00 (payments can be made in cash, debit or credit card)

8. Unused self-addressed envelope, preferably with tracking features, to be used in mailing the new passport to the applicant

B. Renewal of Green Passports, Any Older Passports, and Machine-Readable Passports (Maroon)
  1. Confirmed Online Appointment
  2. Personal appearance
  3. Accomplished application form
  4. Passport with photocopy of data page
  5. Original Birth Certificate from the Philippine Statistics Authority (PSA) to support changes in the passport entries, or if old passport has no complete middle name (if applicable)
  6. For Married Females: Original PSA Marriage Certificate
  7. Original and photocopy of any of the following for confirmation of Philippine Citizenship: valid Canadian Permanent Residency card; valid visitor visa; valid work permit
  8. Processing fee of CAD 93.00

C. Supporting Documents

Pursuant to Section 6 of R.A. 11983, supporting documents will be further required in addition to the basic requirements to prove the applicant’s identity, citizenship and lack of travel restrictions, as specified in the following cases:

1. In case of a Lost VALID/EXPIRED ePassport
  • Original PSA Birth Certificate and photocopy
  • For Married Females: Original PSA Marriage Certificate
  • Police Report
  • Affidavit of Loss executed at the Consulate (Fee: CAD 38.75)
  • Processing Fee: CAD 232.50
2. In case of a Lost Green/Brown/MRP/MRRP Passport
  • Original PSA Birth Certificate and photocopy
  • For Married Females: Original PSA Marriage Certificate
  • Police Report
  • Affidavit of Loss executed at the Consulate (Fee: CAD 38.75)
  • Processing Fee: CAD 130.50
3. In case of renewal of a frequently Lost Passport – TREATED AS A NEW APPLICATION
  • Original PSA Birth Certificate and photocopy

4. In case passport has been mutilated or damaged
  • Original PSA Birth Certificate and photocopy

  • Affidavit of Destruction of Passport executed at the Consulate (Fee: CAD 38.75)
5. In case applicant is a Dual Citizen
  • Natural-born Dual Citizens:
    • Latest-issued Philippine passport; and
    • PSA-issued Certificate of Live Birth or PSA-issued Report of Birth.
  • Dual Citizens who availed of the provisions of Republic Act No. 9225:
    • Latest-issued Philippine passport;
    • Original and photocopy of Oath of Allegiance, Order of Approval, or Identification Certificate issued by a Philippine Embassy/Consulate; or Certificate of Retention/Re-acquisition of Philippine Citizenship or any equivalent document issued by BI; and
    • Canadian Issued government ID
6. In case applicant is a Naturalized Citizen
  • Latest-issued Philippine passport; and
  • Identification Certificate of Naturalization from the Bureau of Immigration, or Certificate of Naturalization issued by the Special Committee on Naturalization (SCN), or copy of the final Court Judgment or Law granting Philippine citizenship.
7. In case applicant is a Filipino citizen by Election
  • Latest-issued Philippine passport;
  • Affidavit of election of Philippine citizenship; and
  • Identification Certificate issued by the BI.
8. If a married woman applicant wishes to revert to her maiden name based on the following:
  • By virtue of death of the spouse:
    • PSA-issued Certificate of Death or Report of Death (ROD) of spouse or apostillized or authenticated Foreign Death Certificate of foreign spouse with English translation, if applicable;
    • PSA-issued Certificate of Live Birth or Report of Birth; and
    • Latest issued Philippine passport (if available).
  • By virtue of an annulment, declaration of nullity of marriage, judicially-recognized foreign divorce, and judicially-recognized divorce under Presidential Decree No.1083 (Code of Muslim Personal Laws of the Philippines):
    • PSA-issued Certificate of Marriage or Report of Marriage (ROM) with annotation reflecting the nullity or dissolution of marriage
    • PSA-issued Certificate of Live Birth or Report of Birth; and
    • Latest issued Philippine passport (if available).
  • For other reasons of reversion:
    • PSA-issued Certificate of Live Birth or Report of Birth;
    • PSA-issued Marriage Certificate
    • Notarized Affidavit of Explanation that includes request for the reversion of maiden name in the Philippine passport or travel document and stating she has not hitherto availed of the reversion;
    • Latest-issued Philippine passport or travel document; and
    • Any existing government-issued valid ID accepted for passport application reflecting the maiden name.

PASSPORT APPLICATION FOR MINORS (17 YEARS OLD AND BELOW)
A. Core Requirements



  • Police Report
  • Affidavit of Loss executed by the parent/s at the Consulate (Fee: CAD 38.75)
  • Processing Fee: CAD 232.50
B. Supporting Documents for Minor New Applications

Pursuant to Section 6 of R.A. 11983, supporting documents will be further required in addition to the basic requirements to prove the applicant’s identity, citizenship and lack of travel restrictions, as specified in the following cases:


1. In case applicant has No PSA Birth Certificate/Report of Birth yet
  • If born in the Philippines – The minor applicant may submit a Certified True Copy (CTC) of Local Civil Registrar (LCR) Birth Certificate authenticated by the PSA.
  • If born abroad – Parents to first file the child’s Report of Birth (ROB). Please refer to requirements for Civil Registration: Report of Birth at the Consulate’s website for the requirements

2. In case of marital minor applicant (with Married Parents as stated in the minor applicant’s PSA Certificate of Live Birth)
  • Valid Philippine passport or other competent proof of identity of the accompanying parent and the copy of his/her spouse’s passport or other competent proof of identity;
  • If the accompanying parent is an alien, the non-Filipino parent must present his or her foreign passport and a copy of the Filipino parent’s Philippine passport or other competent proof of identity; and
  • If accompanied by an authorized adult companion, a Special Power of Attorney (SPA) executed by either of the minor’s parents or his/her legal guardian, as the case may be, his/her passport or other competent proof of identity, and a copy of the passport or other competent proof of identity of the minor’s parents.

3. In case of non-marital minor applicant (with unmarried Parents as stated in the minor applicant’s PSA Certificate of Live Birth)
  • Only the mother applicant shall accompany the minor applicant in the passport application process;
  • A Special Power of Attorney (SPA) executed by the mother will be required if she is not accompanying the minor applicant.The SPA must be authenticated by the Department of Foreign Affairs in the Philippines or by the Philippine Embassy/Consulate abroad; and
  • Passport or Valid Government issued ID of authorized adult companion

4. In case applicant is not accompanied by the parent/s during the application process
  • Applicants must submit a Special Power of Attorney executed by either of the minor’s parents or his/her legal guardian designating an adult companion to assist in the application process. The Special Power of Attorney must be authenticated by the Department of Foreign Affairs or any Philippine Embassy/Consulate abroad;
  • Passport or Valid Government issued ID of authorized adult companion; and
  • Special Power of Attorney must have a copy of parent/s valid ID and/or passport attached.

5. If the applicant is a non-marital minor whose mother is deceased but acknowledged by the father:
  • Personal Appearance of minor applicant and biological father;
  • PSA-issued Certificate of Death or Report of Death of Mother;
  • PSA Birth Certificate with Acknowledgement of Paternity (must indicate the name of the father in the birth certificate);
  • Passport or Valid Government issued ID of biological father

6. In the absence of father, the following may accompany the minor child in the order indicated:
  • Surviving grandparent (submission of PSA Certificate of Death of Mother;
  • Oldest brother or sister, over twenty-one years of age (submission of PSA Certificate of Death of Mother and PSA Certificate of Death of the Grandparent);
  • The child’s actual custodian, over twenty-one years of age (submission of court order for legal guardianship)

7. If the applicant is a non-marital minor and mother is deceased / absent and father is unknown:
  • Personal Appearance of minor applicant and court-appointed legal guardian
  • PSA Birth Certificate
  • Valid Passport or valid government ID of adult guardian
  • Letter of Guardianship issued by Family Court

8. In case none of the above can accompany the minor applicant
  • In default of parents or a judicially appointed guardian, the persons exercising substitute parental authority under Article 216 of the Family Code of the Philippines shall prevail in determining the minor applicant’s companion in passport application, unless otherwise ordered by a competent Philippine court. In case of conflicting claims for custody, a court order shall be required; and
  • In cases involving Filipino minors abandoned abroad, the Consular Official of the concerned Philippine Consulate may initiate the passport application with the best interest of the minor being the primary consideration.

9. In case of Minor Foundlings in the Philippines not for Adoption
  • Personal appearance of minor applicant and the NACC Social Worker or Liaison Officer;
  • PSA-issued Certificate of Foundling;
  • Letter of Guardianship issued by the Family Court; and
  • Valid ID of the guardian.

10. In case of Minor Applicant who is a Prospective Adoptee for Domestic Adoption
  • Personal appearance of minor applicant and NACC Social Worker or Liaison Officer;
  • PSA-issued Certificate of Live Birth or Report of Birth or Certificate of Foundling;
  • Valid passport or valid government-issued ID of authorized NACC Social Worker or Liaison Officer;
  • Certificate of Child Available for Adoption or Deed of Voluntary Commitment / Certificate Declaring a Child Legally Available for Adoption issued by the National Authority for Child Care (NACC); and
  • Official letter or Special Power of Attorney signed by the NACC Regional Office authorizing the Social Worker to process and receive the passport of the minor applicant and must include a copy of valid government-issued ID and/or passport.

11. In cases of Minor Applicant who is an Adoptee through Domestic Adoption
  • Personal appearance of minor applicant and adoptive parent/s;
  • Amended PSA-issued Certificate of Live Birth or Report of Birth, indicating the child’s adoptive name and name of adoptive parents;
  • Passports or other competent proofs of identity of the minor applicant, adoptive parent/s or authorized adult companion; and
  • Certified True Copy of Court Decree of Adoption or NACC-issued Order of Adoption.

12. In case of Minor Applicant who is a Prospective Adoptee for Foreign Adoption
  • PSA-issued Certificate of Live Birth, Report of Birth or Certificate of Foundling;
  • Endorsement from NACC;
  • Certificate Declaring a Child Legally Available for Adoption or Deed of Voluntary Commitment;
  • Placement Authority issued by NACC;
  • Certificate for Issuance of Passport issued by NACC;
  • Travel Clearance Certificate for adoptee;
  • Child Study Report; and
  • Clearance for Intercountry Adoption.

13. In case of Minor Applicant who is an Adoptee through Foreign Adoption
  • Personal Appearance of minor applicant and adoptive parent/s;
  • Amended PSA-issued Certificate of Live Birth, Report of Birth;
  • Passport/s of adoptive parent/s or authorized adult companion. If the adoptive parents are foreign nationals, presentation of their valid foreign passports is acceptable; and
  • Certified True Copy of Court Decree of Adoption.
